The Citizens Health Advisory Council (CHAC) is an open coalition of interested citizens and organizations that advocates for community health and wellness in La Plata County by serving
as a central point for communication and collaboration.

The Citizens Health Advisory Council envisions that….

  • our county is a statewide model for integrated health care in rural/resort areas;  
  • a full system of affordable, patient-centered, high-quality, accessible and cost effective health care is available to all members of our diverse communities;
  • providers and organizations work collaboratively to ensure such a system(s)  is in place; 
  • our health care systems are patient-centered and honor the whole human within the context of family and community,  mind, body, emotions and spirit;  
  • our health care systems  emphasize prevention, healthy lifestyles, social/personal responsibility and disease management; and
  • all factors that influence health are considered including jobs, housing, education, the built environment and our natural resources.
  • Link and collaborate with the La Plata County Health Care Alliance and give input to its activities.   
  • Work to build capacity and momentum towards achieving the goals and projects published in the Community Health Care Capacity Project Plan (released May 2010).   
  • Carry out education and outreach projects that build the public will to achieve our vision including keeping a Web site updated.
  • Continue to host monthly meetings to communicate, collaborate and coordinate across individuals, organizations and sectors.
